Staying out of hospital 2016/17
One of our key priorities is to ensure people aren’t faced with unnecessary delays when they leave hospital.
To address this challenge we are working with the Heart of England Foundation Trust and out of borough hospitals, focusing on a number of ways to improve our ‘Delayed Transfers of Care’ figures.
One of the ways includes developing the Support U Home model in partnership with health colleagues. This new programme aims to prevent hospital admissions, facilitate timely discharge from hospital and increase the quality of residential care provision.
In 2016 the Council’s Reablement service was involved in a review, which was part of a wider transformation called the Advancing Adult Social Care programme. A new operating model for Reablement came into effect in October 2016. This new way of working involves external providers supporting people with less complex Reablement needs and our in-house service supporting people with more complex Reablement needs.
